Telehealth Consent
Effective date: August 13, 2026 · Contact: support@joinvaylen.com
How telehealth works here
You complete a structured health assessment online. A licensed U.S. clinician, licensed in your own state and practicing through OpenLoop Health, personally reviews your information, may message you with follow-up questions, and decides whether treatment is clinically appropriate. Care is primarily asynchronous: it does not necessarily include a live video or phone visit, though one may be offered or required where your clinician or state law calls for it.
Benefits and limitations
- Telehealth improves access and convenience and avoids travel.
- Your clinician cannot perform a physical examination. The evaluation depends on the accuracy and completeness of the information you provide. Inaccurate answers can lead to unsafe care.
- Technical failures may delay evaluation or communication.
- Telehealth is not appropriate for emergencies. If you are experiencing an emergency, call 911.
Possible outcomes
Your clinician may prescribe treatment, recommend an alternative, request more information, or determine that treatment is not appropriate. Approval is never guaranteed, and declining to prescribe results in a full refund under the Refund Policy.
Your rights
- You may withdraw consent to telehealth at any time without affecting your right to future care.
- You may request your records as described in the Notice of Privacy Practices.
- You may seek in-person care instead at any time, and we encourage you to maintain a relationship with a primary care provider.
Acknowledgment
By checking the consent box during the health assessment, you confirm that you have read and understood this consent, the Terms of Use, the Privacy Policy, the Refund Policy (including shipment and cancellation terms) and the Notice of Privacy Practices, and that you consent to receive care through telehealth.
